Salary: £17,400pro rata. Real Living Wage (in line with NLDF Fair Work commitment)
Role Purpose
The Brew & Blether Coordinator will deliver a gentle, person centred homebased support service for adults living with long term conditions, disabilities, or who are on cancer journeys, as well as carers. The service offers four week blocks of practical support and companionship, helping people feel more confident, more connected, and better able to manage at home centred homebased support service for adults living with long term conditions, disabilities, or who are on cancer journeys, as well as carers. The role combines befriending, practical help, empowerment and early intervention support. It also plays a key part in improving mental wellbeing, reducing isolation, supporting smooth discharges from hospital, and linking individuals to sustainable long term community supports.
Key Responsibilities
Direct Support
- Offer supportive conversation, companionship, and emotional reassurance in line with befriending principles.
- Delivery of 4-week blocks of one hour home visits providing practical, light-touch tasks such as dishes, hoovering, dusting, bin removal, and making a cup of tea
- Provide basic IT support to build digital confidence (e.g., online food shopping, accessing information).
- Support individuals to manage basic paperwork or household organisation tasks.
Wellbeing, Empowerment & Pathways
- Build confidence and independence by working at a flexible, person-led pace, “care without the clock.”
- Identify appropriate supports, including Community Connectors, Carer Breather, and other community pathways.
- Support individuals to link with local services and activities to reduce loneliness and encourage community engagement.
Collaboration & Coordination
- Work autonomously across North Lanarkshire, managing your own caseload and rota.
- Collaborate with internal colleagues to support seamless referrals and handovers.
- Contribute to developing a potential hospital discharge referral pathway.
- Work alongside volunteers (administrative or practical), offering guidance and peer support.
Safety, Professionalism & Administration
- Maintain accurate records of visits, wellbeing check ins and outcomes.
- Work to trauma informed principles, and NLDF policies
- Ensure safeguarding, boundaries and confidentiality are always upheld.
- Participate in training, supervision, debriefing and reflective practice.

Person Specification
